Kshara Sutra (Alkaline Thread Ligation)

Kshara Sutra is a globally recognized, minimally invasive Ayurvedic para-surgical procedure. A specialized surgical thread (Sutra), repeatedly coated with herbal alkaline ashes (Kshara) and latex, is used to ligate and simultaneously cut, clean, and heal severe fistulas and hemorrhoids.

Clinical Procedure
1. Preparation (Purvakarma)
- • The fistula tract is probed under local anesthesia to determine its path. A specialized thread (coated 21 times with Snuhi latex, Apamarga ash, and Haridra) is prepared.
2. Main Action (Pradhana Karma)
The Kshara Sutra is passed through the entire fistula tract and tied securely. The chemical action of the thread slowly cuts through the tissue while simultaneously healing it (about 1 cm per week).
3. Post-Therapy (Paschatkarma)
- • The thread is changed weekly by tying a new thread to the old one and pulling it through, until the tract is completely cut out and healed.
- • Daily warm sitz baths (Avagaha Sweda) are mandatory.
Patient FAQs
Does the thread hurt when it cuts?
The initial application is done under local anesthesia. During the week, you may feel mild discomfort or a pulling sensation, but it is highly manageable with oral Ayurvedic pain relievers and daily warm sitz baths. Patients can go to work normally.

Contraindications
- Fistulas associated with strictures, severe Crohn's disease, or advanced malignancies
- Extremely high tracts extending into the abdominal cavity
- Pregnancy
