Virechana is one of the five primary Panchakarma therapies, focusing on the medically induced purgation of the lower gastrointestinal tract. It is the treatment of choice for eliminating excess Pitta dosha and toxins (Ama) accumulated in the liver, gallbladder, and small intestine.

Clinical Procedure

1. Preparation (Purvakarma)

  • Deepana and Pachana: Herbs to correct digestion for 3-5 days.
  • Snehapana: Internal oleation (drinking medicated ghee) in increasing doses for 3-7 days until saturation signs appear.
  • Abhyanga and Swedana: Full body oil massage and steam for 3 days to liquefy toxins and move them to the gut.

2. Main Action (Pradhana Karma)

On the designated morning, a specific purgative medicine (e.g., Trivrit Lehyam or Castor oil formulation) is administered. The patient will pass multiple loose stools throughout the day, systematically eliminating the accumulated toxins.

3. Post-Therapy (Paschatkarma)

  • Samsarjana Krama: A strict graduated dietary protocol starting with rice water (Peya) and slowly moving to solid food over 3-7 days.
  • Complete physical and mental rest.
  • Avoidance of cold drafts, travel, and strenuous exercise.

Patient FAQs

Will I feel weak during the purgation day?

While you will pass multiple stools, Ayurvedic purgatives are designed to expel only toxins (Mala) and morbid doshas, not healthy fluids. You will be continuously monitored and given warm water. Most patients feel remarkably light and energetic after the process, not weak.

Why do I have to drink ghee before Virechana?

Drinking medicated ghee (Snehapana) is crucial. The fat acts as a solvent, absorbing deeply lodged, fat-soluble toxins from the tissues and drawing them into the gastrointestinal tract so they can be flushed out during purgation.
